Obtain tax forms and information and find the local office nearest you. The Internal Revenue Service has a centralized tax exemption office at 201 West Rivercenter Blvd., Covington Kentucky 41011. All I.R.S. tax exemption forms can be ordered by phone at 1-800-TAX-FORM, or downloaded from the I.R.S. web site at www.irs.gov. It’s exempt organization question line is 877-829-5500.
