  •   Tax Information for Tax Exempt Entities. Order forms and ask advice about applying for exempt status and completing tax returns. Obtain tax forms and information and find the local office nearest you. The Internal Revenue Service has a centralized tax exemption office at 201 West Rivercenter Blvd., Covington Kentucky 41011. All I.R.S. tax exemption forms can be ordered by phone at 1-800-TAX-FORM, or downloaded from the I.R.S. web site at www.irs.gov. It’s exempt organization question line is 877-829-5500.
